"Five loops" of smart wards escort medical safety
Date:2020/05/17

On September 28, 2018, Peking University Shenzhen Hospital, Shenzhen Nursing Association and Shenzhen Women’s Medical Doctor Association jointly held the 2018 China (Shenzhen) Smart Hospital—Medical AI Clinical Application Summit themed as “Innovation · Integration · Intelligence”. Peking University Shenzhen Hospital once held the "Intelligent Nursing Summit” in 2016, and now, after two years of exploration, officially is upgraded as a smart hospital.

At the summit, Chen Yun, President of Peking University Shenzhen Hospital, gave a detailed report on the construction of a smart hospital and defined a smart hospital as: “Our construction of a smart hospital takes the hospital as the platform, patients as the center, and application technology as the carrier, so as to better the medical services for patients, improve the medical process, and enhance the efficiency of operating the hospital”.

Most of the core products in the visit area of the smart ward of Peking University Shenzhen Hospital are innovatively developed by Lachesis based on the hospital's actual needs for clinical applications. If you accidentally miss the opportunity to visit the Smart Ward in person, the original short video below will give you some idea.

Lachesis takes the whole process of nursing service as the main line and patients’ safety as the core, builds five safety closed loops around the clinic, guarantees the safety of patients, improves the working efficiency of medical staff, and upgrades the level of medical services.

Closed loop of ward quality management

Since the admission of patients, medical-grade intelligent equipment is applied in the management work, including the real-time verification of patients’ identity and the execution of medical instructions, the establishment of a risk prevention and control platform, the early identification and intervention of critical patients, the real-time reminders of daily work nodes, the real-time monitoring of the nursing process, and the immediacy and continuity of rehabilitation guidance. At the same time, a workflow-related knowledge base is established to provide reference for first-line nurses, guide nurses to complete tasks such as assessment, observation, treatment and rehabilitation according to plans and specifications, and construct an inpatient safety management model led by nurses, coordinated by doctors, and participated by patients and their families.

The closed loop of ward quality management mainly includes the following aspects:

Patient personal identity verification (PIV) management: After admission, each patient’s information will form a two-dimensional code and be put in the wrist strap, so that it has the unique role of verifying the patient's identity. Nurses can use Nurse Digital Assistant (NDA) to scan the QR code and confirm the identity of patients.

Nursing clinical path management: Patient information enters the nursing clinical path system. The nursing clinical path is synchronized with the medical path and runs through the entire hospitalization process of patients. After a nurse starts the path system menu, he or she can perform step-by-step standardized operations according to the items listed in the system, receive real-time reminders of key tasks, and monitor the nursing process in real time. Besides, the system is provided with a strong nursing knowledge base to help junior nurses effectively enhance the their capabilities and provide homogeneous care for inpatients.

Safety management for special patients: Patients that have special conditions and require bed rest can be managed by the non-contact sign collection system. When a patient leaves the bed, the system will automatically issue an alarm and the nurse will intervene in time. For patients with a high risk of pressure sores, their mobility on bed should be monitored and personalized rollover management should be provided based on the results. For patients with special risks such as suicide and wandering away, electronic wrist straps can be used for accurate positioning. If a patient goes beyond the scope of activities, the system will immediately issue an alarm to ensure the timely intervention of nurses and reduce the occurrence of accidents.

Early identification and management of conditions of critical patients: Vital sign data is measured by wearable devices and automatically uploaded to directly generate a temperature sheet. This reduces errors and omissions caused by repeated transcriptions by nurses, and saves nurses the time to draw up temperature sheets. In the management of patients, potential critical illnesses are increasingly concerned by clinical nurses. Based on the obtained vital signs, the system can automatically analyze the patients’ mews score, timely identify the "potentially critically ill" patients, and facilitate nurses to take effective and reasonable therapeutic intervention as soon as possible.

Closed loop of drugs management

This function integrates the intelligent medicine cabinet, medicine distribution robot, infusion monitor and NDA, etc., realizes the traceability of the information on the entire process including issuing doctor's advice, implementing medication and ending medication, builds a safe closed-loop of drugs management, and improves the safety of medicines.

Lachesis intelligent ward medicine management system connects the hospital pharmacy and doctor's instructions. For the temporary medicines of patients in the ward, after the nurses select the medicines to be taken according to the doctor's instructions, the indicator light of the medicine box will point out the location of the medicines, so as to discharge medicines as required in a fast and intelligent way and prevent taking more, less or wrong medicines. The system can intelligently manage the location, validity period and batch number of each drug, and intelligently determine and discharge the near-expiration drugs, thus avoiding unnecessary waste caused by expired drugs. Furthermore, functions of power-on self-test, temporary statistics and automatic inventory can shorten the time of manual inventory check, verification and tabulation.

When medicines need to be replenished in emergency situations, pharmacy personnel will put the medicines into the medicine box of the robot that controls the distribution of medicines, and the robot will automatically navigate, avoid obstacles and deliver the medicines to the nurse station quickly and safely.

When a nurse gives infusion to a patient, the nurse can hang an intelligent infusion monitor on the patient's infusion stand. Based on the microgravity sensor and a number of patented technologies and patented algorithms, the monitor can accurately supervise the remaining amount of infusion and transmit the data to the nursing background through Bluetooth .

Nurses can use a large screen, a computer, or portable NDA at the nurse station to remotely check the drip rate and residual volume of patients' infusions. The system will issue intelligent reminders on how many minutes away it is to connect the bottle and remove the needle, or whether there is any abnormality during the infusion.

Closed loop of perioperative period management

By scanning NDA barcodes, patient information is confirmed and shared in real time, thus realizing closed-loop management of surgical patients in the whole process including transfer handover, security verification, specimen verification, surgical instrument inventory and consumable traceability. Standardized management is implemented in the key nodes of the entire process before, during and after the operation, which has improved the administrative capacity of the core system of perioperative safety management, reduced the incidence of perioperative errors, and achieved the goal of safe management of surgical patients.

The distribution robot in the operating room can facilitate to place instructions and realize automatic logistics distribution of high-value consumables and instruments between the operating room and the secondary warehouse, avoid interrupting surgical operations due to the need to take medicine and instruments during operation, and shorten the waiting time during operation.

Closed loop of device management

Emergency equipment management: The distribution of first-aid equipment in the whole hospital is clarified by electronic tags, especially the location and statistics of ventilators and defibrillators, which facilitates the coordination in the whole hospital in special circumstances. Nurses can quickly identify the specific locations, quantity and status of the first aid equipment in their department, find the first aid equipment in time, and ensure that the emergency treatment items are in place in time.

Closed loop of outpatient management

This function is connected with the hospital's registration system. Through indoor map technology and positioning technology, the whole process of patients visiting doctors is intelligently guided, so as to reduce the patients’ length of stay in the hospital, improve the patients’ medical experience, and upgrade the hospital's service efficiency.
